Maw-cult Fanatics is the attrition battle formation for Ogor Mawtribes in Age of Sigmar 4th edition, from Battletome: Ogor Mawtribes (July 2026). It is the formation that makes an already tough army genuinely hard to remove.

The formation ability is Grub's Up, Mateys!, used once per turn at the end of any turn. Every friendly Ogor Mawtribes unit that has used the army's Eat 'Em Alive battle trait at any point in the battle heals a small amount.
The phrasing matters. This is not a heal for units that fed this turn: it is a heal for units that have ever fed. The pool of eligible units only grows, so a turn-five trigger can be repairing most of your army at once.
Eat 'Em Alive is used at the end of a turn by an Ogor unit that fought that turn, so the formation asks you to get every unit into a fight once, early, and then keep them alive. A unit that never fights never joins the healing pool, which makes bubble-wrapped backline units the weakest fit for this formation.
The best targets are the models with the largest health pools and the highest points cost, because a fixed heal on a big monster is worth far more than the same heal spread across cheap infantry.
Players who want a grinding, hold-the-middle Ogor army built on monsters and elite infantry, and who are happy to win slowly. It is the most forgiving of the four current formations to pilot, because it converts mistakes back into wounds.
Pick Maw-cult Fanatics when your list is monster-heavy or built on expensive infantry that you cannot afford to replace, and when you expect a long game rather than a decisive alpha turn. If your army is wide and cheap, Hunger-filled Tribe converts kills into board control more usefully than a small heal spread thin.

No. The trigger is having used Eat 'Em Alive at some point in the battle, so a unit joins the healing pool once and stays in it. Getting every unit one fight early is the way to build the engine.
The ones with the biggest health pools and the highest cost, because a fixed heal repairs a larger share of value on a Stonehorn or a Thundertusk than on cheap infantry.
